Choice of Services
Whether you are thinking about going into a nursing home or you are being
discharged and transferred to another nursing home because the nursing
home you are in is closing, you have a choice of services. If you are
eligible for nusing facility level fo services, you can choose services
in a nursing home or you can choose services at home or in the community.
If you decide to stay in the community, you can receive the following
services at home and in the community, depending on your needs:
- Case Management
- Homemaker
- Attendant Care
- Respite Care
- Adult Day Services
- Home Delivered Meals
- Home Modifications
- Adaptive Aids and Devices
- Other services to help you stay home or in the community
Who can I call for information?
Your Senior Care Team is available to assist you. The members of the
Senior Care Team include:

You have the right to attend a relocation-planning meeting to
determine your needs and desires in selecting another place to live. You
have the right to have anyone of your choosing to attend the meeting with
you.
You have the right to be involved in the selection of your next
location and ot make the final decision about where you want to live.
The current facility must help you find a place that can meet your needs.
You cannot be tranferred to a place that is not able to meet your needs.
You have the right to receive a written notice that your nursing
home is closing on a form prescribed by the Indiana State Department of
Health at least 30 days in advance. If the notice is not in response to
a complete facility closure, you ahve the right to appeal.
The facility must assure that all of your medications, medical
records, personal funds, and personal possessions are transferred with
you.
You have the right to consult with an Ombudsman at any time.
Services of the Ombudsman are free and confidential. The phone number
of your Local Ombudsman should be posted in the facility or you may call
the State Ombudsman at 1-800-622-4484.
What if I am currently in a nursing home and the home is closing?
If you are currently in a nursing home and you receive a notice that
the facility is closing, you may be able to receive services that can
help you return home or to the community instead of going to another nursing
home. If you choose to go to another nursing home, your local Long Term
Care Ombudsman will help you and your loved ones find another nursing
home for you and help you move smoothly.
If you choose to go home or into the community insted of going to another
nursing home, the following things will happen:
- You will be able to choose a case manager in your area.
- Your case manager will work with you and your loved ones to determine
if you are eligible, and to develop a Plan of Care. Your Plan of Care
defines what services you will need in order for you to live in your
home and/or community safely.
- Your case manager will help you choose providers of these services.
- Your case manager will follow up with you periodically to make sure
you are getting the services you need and will work with you to make
any necessary changes to your Plan of Care, such as changes in you services
or service providers to assist you in safely remaining in your home
or community setting.
The Ombudsman is an advocate for nursing home residents, and is available
to provide free, confidential assistance at any time. He or she will assure
that your rights are protected during any transfer, discharge or relocation,
and can assist you in the relocation planning meeting, and in selecting
a relocation site that will meet your needs.
